UPSC CMS Application Form 2021: The UPSC CMS 2021 application was activated on July 7 with the release of an official notification. The UPSC CMS 2021 online application deadline is July 27. UPSC CMS Application Form 2021: Documents Required

Certain documents and details must be prepared before filling out the CMS application form. The following documents/details are required:

• Bank details for online transaction
• Mark sheet and admit card of Class 10 and 12

UPSC CMS Application Form 2021: Steps to Fill

The UPSC CMS application has two parts. The steps to fill both the parts are given below:

CMS Application Part 1

Candidates can apply for the UPSC CMS at the UPSC official website. The steps are as follows:

UPSC CMS Application Form: Part II

1. Candidates must go to the UPSC official website and click on the “Click Here for Part II” link.
2. After clicking the link, a new page will open in which candidates must enter their registration ID as well as their date of birth before clicking the Submit button.
3. The Combined Medical Services exam part-II application form will appear on the screen after you click the button.
4. Candidates must fill out the required information and then click the “Submit” button.
5. Candidates’ CMS exam application process will be completed upon submission of the part-II registration form.

UPSC CMS Application Form 2021: Required Details

• Name
• Gender
• Date of birth
• Nationality
• Marital status
• Parents’ names
• Email ID
• Postal address
• Educational qualification, etc.

UPSC CMS Application Form 2021: Application Fee

After filling out the personal information in part 1 of the UPSC CMS application form, candidates must pay the application fees. Please keep in mind that there is no application fee for candidates who fall into the SC or ST categories. Female candidates are also exempt from paying examination fees.

The application fee for candidates in the General and OBC categories is Rs 200. The application fee can be paid online or by SBI Bank Challan.

Mode of Payment of Fees: Candidates can pay the application fee both online and in person. The following information is provided:

1. Online Mode: Candidates can pay the UPSC CMS application fee online using any credit/debit card or internet banking.
2. Offline mode: Candidates may pay their application fee in cash. They can do so by paying in cash at any SBI branch. During part II registration, the candidate must print the system-generated pay-in-slip and deposit the fee at the counter of SBI Branches on the next working day only.
3. Candidates should be aware that the ‘Pay by Cash’ option will be disabled one day before the application process closes. Candidates who generated their pay-in-slip before the process was deactivated, on the other hand, can make the payment at any SBI branch during banking hours on the closing date.

After paying the application fee, candidates must click the “Continue” button and select a Combined Medical Services exam centre. Following that, candidates must upload their scanned photo and signature in accordance with the above-mentioned specifications.
